Position Information:
The Pulmonary Navigator works in collaboration with the multidisciplinary care team to assess and coordinate chronic disease management for patients with chronic pulmonary diseases. The Navigator initiates and accepts patient referrals, assesses healthcare needs, identifies community and medical resources, and manages assigned patient population. Responsible for maintaining the COPD database on a daily bases and recommending trends for process improvement. Must possess strong customer service, interpersonal and time management skills.

Experience Requirements:
3 Years Clinical - Respiratory Therapy Required - And Analysis - Data And Clinical - Research And Process Improvement Preferred

Education Required:
Associate's Degree Respiratory Therapy Required - And Bachelor's Degree Respiratory Therapy Preferred

Licensure/Certification Requirements:
Registered Respiratory Therapist Required - And Pulmonary Disease Educator from AARC Required - Within 1 Year of Hire And Tobacco and Smoking Cessation Training from AARC Required - Within 1 Year of Hire

Hours of Work:
Monday-Friday 8hr shifts

Weekend Requirements:
As Needed
