The Clinical Documentation Specialist is responsible for leading chart review and abstraction for Medicare Risk patients. Reporting to the Director of Risk Adjustment Education, the Clinical Documentation Specialist supports all aspects of chart review and abstraction to ensure accurate and complete coding and documentation of encounter data. The Clinical Documentation Specialist will demonstrate in-depth knowledge of clinical documentation requirements; will review patient charts in the EHR and evaluate gaps; will identify patients eligible for meeting exception and exclusion criteria; and will manage all patient-level submission activities through a third-party platform.ESSENTIAL DUTIES & RESPONSIBILITIES: The duties listed below are intended only as illustrations
