The incumbent serves as Coordinator for the Community Residential (CRC) and Nursing Home Care Programs and assumes the technical and administrative responsibility for the management of the CRC Program, and the Adult Day Health Care Program within the Social Worker Service at the Veterans Healthcare System of the Ozarks in Fayetteville, AR. The duties of the Social Worker Community Care Program Coordinator in the Social Work Service include but are not limited to: The incumbent will prepare these Community Care Programs to adhere to all current directives of VHA and meet current JOINT COMMISSION standards, State licensure standards and VA-OIG expectations. The incumbent will conduct ongoing intensive reviews and appraisals of the programs to ensure the accomplishment of objectives and adherence to standards set by governing bodies. The incumbent is in charge of program development, recruitment, and development of Nursing homes, residential care homes, and Adult Day Care facilities. Continuous monitoring and improving of the efficiency, effectiveness, timeliness and quality of services offered within the Community Care Programs. This includes the development and monitoring of quality indicators. Identification of areas for improvement, gather relevant data, assess the data, develop and implement ideas for improvement and evaluate efficacy of improvements in the organization of clinical and administrative social work functions within the Community Residential Care Program area. Recruit new Community Residential Care homes, Assisted Living Facilities and Home Health Agencies as the need arises. Maintains a calendar of when inspections are due and coordinates, schedules, oversees inspections and completes a final reports. Monitors the follow-up of corrections required of a VA community facility/agency as a result of the formal inspection conducted by the Community Residential and Nursing Home Care Program Inspection Team of which the incumbent will serve as the Inspection Team Leader. Functions as the professional representative of the Veterans Healthcare System of the Ozarks (VHSO), Social Work Service (SWS), and his/her program providing public relations and community education regarding the Community Residential Care Programs. This includes Federal, State, County and City government offices, and private non-profit services, agencies and organizations. The incumbent is the liaison with community partners, nursing home facilities and agencies that have contracts and formal agreements with the VA to provide services to the Veterans.
