Silver Maples was founded in 1997 with a goal of addressing the needs of active seniors by developing a community which fosters its residents' independence, security, wellness, dignity, privacy, comfort and well-being. There are 17 Independent Living Villas, 77 Independent Living Apartments and 58 Assisted Living Apartments nestled in serene woods on over 17 acres just minutes away from Chelsea's downtown area, fifteen minutes from Ann Arbor and a short drive from Chelsea. Silver Maples of Chelsea is hiring a full time Clinical Coordinator to work in our Meadows Assisted Living Department. The Clinical Coordinator must be able to assess resident needs in the Assisted Living setting and be able to plan and implement resident services; which are documented and communicated via the “Service Plan” as mandated by the HFA regulations. Must effectively interact with residents, families, and other members of the Silver Maples team to ensure that quality care and services are maintained. This position reports to the Assisted Living Director and serves as “back up” to the Director when necessary. Ensures compliance of State Rules and Regulations of Homes for the Aged licensing.
Other Duties and Responsibilities:
Participates in the admission of new residents and in the coordination of medical care for residents. Provide support to all Silver Maples residents. (IL, AL, Villas) if indicated.
Responsible for non-urgent and emergent response to all residents. Demonstrates ability to handle emergency situations in professional manner. Promotes AL staff in providing comfort and well-being to the residents, acknowledging physiological and psychological needs.
Demonstrated correct and safe technique in the use of equipment according to specific product information; follow appropriate procedure for obtaining, returning, cleaning, and disposing of equipment and supplies.
Assists residents, family members, or other clients with concern and empathy, and respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
Must be available “on call” via telephone. On call is shared with AL Director.
Oversee and ensure that physician orders are followed pertaining to individual residents. Must be able to read, interpret, and follow up on abnormal lab work in conjunction with the physician.
Must have medical knowledge and ability to speak knowledgeably with physicians, social workers, EMT’s, discharge planners, families, emergency room personnel, and other persons involved with resident care. Must be able to accurately read, interpret, and process physician orders.
Must have ability and knowledge to complete long term care insurance forms as requested by resident and individual insurance companies.
Serve as point person/liaison for outside services related to resident care. (i.e.: Hospice, Home Care, Personal care, PT, OT, Pharmacy services.)
Must learn, utilize, train, implement, and trouble shoot the electronic record system. (Point Click Care).
Responsible for narcotic records including obtaining accurate prescriptions, re-orders, overseeing narcotic counts, and ensuring proper storage of and disposal of narcotic medications.
Provide input and assistance to AL director regarding employee hires, discipline and evaluations.
Registered Nurse (preferred) or Licensed Practical Nurse in the State of Michigan, with current license.
One to three years geriatric experience, memory care experience preferred.
Must demonstrate good written and verbal communication skills in the English language, necessary to communicate and work effectively with various levels of staff, residents, family members, and the public.
Must have knowledge and appreciation for the social/life history of the residents.
Computer skills a must.
Knowledge of State Rules and Regulations related to the care of seniors.
