The Department of Pharmaceutical Services provides comprehensive pharmaceutical care services to Vanderbilt University Hospital (VUH), Monroe Carell Jr. Children's Hospital at Vanderbilt (Children's Hospital), Vanderbilt Psychiatric Hospital (VPH) and The Vanderbilt Medical Group (VMG).
This patient-centered service is provided through the Patient Care Center organization and clinic structure.
The role of the Pharmacy is to promote safe and effective medication therapy for all patients and to provide oversight and leadership of the medication use system.
This Clinical Pharmacist will be a member of the Clinical Pharmacy Team. This pharmacist will provide comprehensive pharmacy services to adult cardiology patients at Vanderbilt University Medical Center. Responsibilities will include provision of direct care through multidisciplinary interactions, medication reconciliation, optimizing pharmacotherapy, order verification, and patient counseling. The pharmacist will provide care for a variety of cardiology specialties, including care of patients with a Ventricular Assist Device. The qualified candidate will perform targeted drug therapy monitoring, medication use evaluations, and quality improvement projects consistent with Vanderbilt University Medical Center goals. Excellent teaching skills are essential, as the clinical pharmacist will be responsible for staff development, precepting and mentoring pharmacy students and residents, and providing formal and informal instruction to nursing and medical staff. Collaboration and integration with pharmacy, nursing and medical staff, and excellent communication skills are essential.
Completion of an ASHP-accredited PGY1 residency or equivalent
